Bet Tzedek Transitions Project

An innovative initiative to provide holistic and coordinated services for adults with intellectual/developmental disabilities and their family caregivers in Los Angeles County. More than 85% of adults with intellectual/developmental disabilities are still living home with their aging parents, and these families need support, information and guidance as they plan ahead for the future.

Bet Tzedek has developed a new Letter of Intent for families to fill out, along with a companion form for their adult relative with Intellectual/Developmental Disabilities.
